The Mortality Review System (MRS) can help eliminate mortalities that re caused by preventable medical errors through processes or systems of care failures. MRS identifies these process and system failures that can be improved to reduce the mortality rate. Each inpatient death is independently reviewed using a mixed‐method process. Every patient’s case and issues identified are recorded in a central registry that can be later discussed in a large group of multidisciplinary setting and determine preventability of deaths caused by these issues.
